Empowering Indigenous strengths to better health and wellbeing: The co-design and evaluation of a sustainable practice nurse health and wellbeing funding model for chronic disease management at a rural Aboriginal Health Service
This project proposes an Indigenous-led, community co-design and evaluation of a sustainable Practice Nurse-Model of Care (PN-MoC) for chronic disease management at a rural Aboriginal Health Service (AHS). The PN-MoC will focus on the values and needs of Indigenous Australians to redefine the way health care is provided, ensuring equitable access and culturally acceptable health outcomes.
Evaluation of the PN-MoC will determine patient and practitioner satisfaction, financial benefit, sustainability, and impact on patient quality of life. Evaluation of the PN-MoC will also further inform AHS practice and future Indigenous health policy, reducing the unacceptable health inequalities and inequities that Indigenous people experience. Ultimately, the project will empower Indigenous self-determination by establishing community-driven healthcare outcomes.
